After enrolling, and being assigned a VA center location, you’ll be give a team — yes, an entire team — of professional health care providers (and even possibly family members, too).

Here are a few examples of the members that can be part of your team:

– A primary care provider (doctor, nurse practitioner, physician’s assistant)

– Clinical pharmacist

– Registered nurse care manager (RN)

– Licensed practical nurse or medical assistant and clerk

– Social workers or other specialists
